Prescriptions

In SMPCC Prescriptions can be obtained through a number of different methods. 

1. By providing a written request to the Reception Office. For your convenience you can print off this form, on which you must for each medicine you require write out the:
   Name of the medicine
   Dose required
   How many time per day you take it
   
   And send or hand deliver this to the Reception office.
   
   
2. Ring the Reception between 9.30am and 12.30pm Monday to Friday.  Please ring 021 430 1745 and our Reception Team will look after your request.

Please allow 48 hours before collecting the prescription.

For most pharmacies, we can offer delivery of your prescription direct to your nominated pharmacy, but 72 hours notice must be given if you wish to avail of this service.

If you wish to have your prescription delivered to your pharmacy of choice, you must sign a consent form in advance nominating the pharmacy.

Also, if you are unable to collect a prescription from the Reception and wish another person to do this for you, please note that this must also be arranged in advance, along with written consent nominating the person who can collect prescriptions on your behalf.
